Offer description

The post involves working alongside the Clinical Lead for Enhanced Care and the team coordinator to support the enhanced care team on a rota basis (8am-6pm). The role requires professional leadership, clinical assessment and coordination of care for patients needing 1-1 supervision, as well as quality improvement projects and training delivery across the trust.


Main Tasks

* Provide professional leadership to the Enhanced Care Support Worker (ECSW) team.
* Manage the A-level student enrichment programme, engaging with enrichment leads in local colleges and supporting spread across localities.
* Conduct holistic clinical assessments for patients requiring advanced 1-1 supervision, in conjunction with the Clinical Lead.
* Allocate an ECSW to work with ward teams, delivering enhanced care and therapeutic interventions in partnership with patients’ families/carers.
* Demonstrate delivery of up‑to‑date practices, policies and protocols in line with best practice within national dementia strategies.
* Lead and support Quality Improvement projects across the trust, taking lead on QI projects when required.
* Collaborate with the Dementia Lead matron to facilitate ongoing professional development and training.
* Provide training trust‑wide in accordance with national guidance in the Dementia Strategy, the Butterfly scheme and Johns Campaign.
* Engage with voluntary sector and community organisations.


Clinical Duties

* Support care provision for patients on wards requiring enhanced 1-1 care via allocation of ECSWs.
* Take active role in risk assessment and planning of patient care with ward teams, implementing risk minimisation strategies.
* Maintain accurate records reflecting changes in treatment and interventions, in line with the Enhanced Care Policy.
* Collaborate across integrated teams to facilitate timely patient/carer interventions and effective transfer of care.
* Ensure appropriate safeguards to protect patients at risk of abuse, including advice on Mental Capacity assessments and DOLS.
* Act as patient advocate, supporting and promoting their involvement in decision making.
* Provide bespoke on‑the‑job coaching and training to ward teams on individualised and therapeutic interventions.
* Establish and maintain effective communication and relationships with multidisciplinary teams, carers and other care settings.
* Offer relevant advice and information to professional colleagues, patients, carers and the public.
* Promote clear information, education and advice for patients, carers and families.
* Assist in recruitment and selection of junior staff.
* Act as an innovative and enthusiastic role model, providing leadership, guidance and advice on operational and professional issues.
* Attend and participate in meetings as required, providing feedback and disseminating information.


Professional Duties

* Act as a role model for quality care, promoting evidence‑based interventions.
* Act as a patient advocate at all points in the patient’s pathway.
* Maintain the NMC/HCPC Code of Conduct at all times.
* Adhere to national, local and professional standards, guidance and directives.
* Provide mentorship and preceptorship as required.
* Take responsibility for own continuing professional development, identifying needs and maintaining portfolio in accordance with registration requirements.
* Support development of other team members.


Managerial Duties

* Deputise for the Clinical Lead for Enhanced Care and support Team.
* Recognise, prevent and manage conflicts, reporting to the Clinical Lead if unresolved.
* Work with ward sisters/charge nurses, college enrichment leads and community teams to ensure quality care delivery.
* Proactively develop the enhanced care and support team, supporting service development and quality improvement with new ideas.
* Support a culture where individuals can report incidents, and learning occurs from incidents and complaints.
* Build relationships with MDT members across the trust.


Financial Duties

* No financial duties are required.


Managing Self

* Participate in regular supervision.
* Attend all mandatory training.
* Participate annually in identifying, developing and agreeing own development plan with Line Manager using the Trust Appraisal.
* Comply with all Trust policies, procedures and protocols.
* Adhere to the Trust’s Equal Opportunity Policy.
* Seek advice and support from Line Manager when necessary.
* Maintain professional conduct and appearance at all times.
* Ensure maintenance of Professional Registration.
