What you will do:
* Conduct essential assessments for minor equipment and adaptations such as rails and galvanised rails.
* Recommend simple adaptations by using your Trusted Assessor skills at Level 3 and Level 4.
* Collaborate closely with the Adult Social Care Team to ensure high-quality care and support for adults in need.
* Maintain and update accurate records of assessments and recommendations.
* Travel across different locations as required; therefore, a driving licence is essential.
