About us
Connnnect Healthcare Rotherham Community Interest Company (CIC) is a federation of 28 GP practices in Rotherham. The organisation is led by a Board of representatives from general practice in the Borough, with executive leadership from a Medical Director on a part-time basis.
The organisation currently delivers extended access primary care services in Rotherham and a Physio First service in partnership with the local acute and community trust, the largest of its kind currently operating. In addition, the organisation is supporting the development of primary care networks and manages delivery of training and development to large cohorts of the general practice workforce.
Our Vision
We are a GP-led organisation and aim to provide services for direct patient care and to support GP practices. We will be a patient‑centred, working collaboratively across general practice and with partners to provide high‑quality services, positive working experiences for our staff, and to secure the sustainability of general practice in Rotherham
Qualifications
* GP with full GMC registration and license to practice
* Experience of working in a GP training practice using SystmOne
Experience
* Experience of working in urgent care and/or a primary care environment
* Experience of dealing with a range of clinical conditions
* Experience of providing home visits
* Proven ability to evaluate the safety and effectiveness of own clinical practice.
* Understanding of evidence‑based practice
* Evidence of working autonomously, with some support and supervision
* Experience in triage
* Knowledge of Quality and Outcomes Framework.
* Knowledge of public health issues in the local area
* Awareness of issues within the wider health arena
* Knowledge of health‑promotion strategies
Clinical Knowledge and Skills
* Ability to effectively triage, assess and diagnose
* Requesting pathology tests and processing the results, advising patients accordingly
* Assessing and managing acute deteriorations/exacerbations of Long‑Term Conditions
* Understands the importance of evidence‑based practice
* Ability to assess and manage patient risk effectively and safely.
* Broad knowledge of clinical governance
* Ability to work within own scope of practice and understanding when to refer to GPs
* Knowledge of public health issues in the local area
* Knowledge of health promotion strategies
Other Skills
* Excellent communication skills (written and oral) including recording of accurate clinical notes.
* Strong IT skills and competent in the use of Microsoft Office and Outlook
* Effective time management (Planning, Organising and Prioritising), with excellent problem solving and analytical skills
* Ability to work autonomously, and as part of a multi‑disciplinary team
* Ability to listen and empathise whilst being honest and caring
* Ability to follow clinical policy and procedure with and understanding of the audit process
* Knowledge of General Practice Clinical Systems e.g. SystmOne
