This is an exciting time to join Derbyshire Healthcare NHS Foundation Trust as a Nurse or Occupational Therapist!

The High Peak Older Adult Community Mental Health Team (CMHT) and Living Well Team seek an enthusiastic Band 6 Older Adult Specialist to join their friendly team based in Swadlincote.

Main duties of the job

The successful candidate will have experience working with older adults with mental health conditions in a community setting.

This role involves supporting introductions into the Living Well Team, assessing needs, and designing person-centred support to help older adults live their best lives.

The role supports community integration, working with colleagues across agencies to provide timely, appropriate services.

The post is supported by the Older Adult CMHT and involves working into the Living Well Team to deliver integrated care.

Living Well is a locality-based, multi-agency team offering seamless support, especially for those who haven't accessed secondary mental health services before.

The successful candidate will need the ability to travel across the geographical area.

Detailed job description and main responsibilities

The post holder will support a flexible, responsive local service, developing an Integrated Wellbeing Pathway aligned with the Community Mental Health Framework. Collaboration with colleagues, partners, and the community is essential.

The role includes conducting comprehensive person-centred psychosocial assessments, risk assessments, and developing care plans. Building therapeutic relationships and empowering individuals are key.

The practitioner will motivate and support individuals to improve mental health, develop self-care, resilience, and utilize community resources. Building relationships with community partners and listening to lived experiences are vital for service development.

Essential criteria

* Diploma in Nursing/Degree in Mental Health Nursing or Degree in Occupational Therapy

Desirable criteria

* Assessor qualification
* Further relevant training

Experience

* Post-qualification experience in relevant field
* Experience working with older adults with mental health conditions
* Community working experience
* Ability to meet travel requirements
* Good communication skills
* Good IT skills
* Ability to work independently and in a team

Desirable criteria

* Linking with partner agencies experience
* Understanding of the Living Well programme
* Good time management
