Rehabilitation Assistant - Neuro ESD

An exciting opportunity has arisen for an enthusiastic and motivated Rehabilitation Assistant to join the Neuro Early Supported Discharge Team.
The successful candidate would join our team from 3rd August 2026.
The Neuro ESD Team is a multidisciplinary team providing specialist intensive rehab to clients with acute stroke and other neurological conditions on discharge from hospital or as community admission avoidance. The post holder will be working alongside a team of Physio's, Occupational Therapists, Speech & Language Therapists, Clinical Neuropsychologist, Rehab Assistants & Admin, working the Southampton and West Hampshire areas.
The team provides six-week intensive neurorehabilitation to individuals with a stroke or Neurological diagnosis, who have been newly discharged from hospital or are at risk of admission into hospital. Care is delivered in a client's own home or alternative community setting.
Working Pattern: Monday - Friday with the expectation for some weekend working.
A full and valid UK driving licence and access to a vehicle for work purposes is required for this role.

Main duties of the job
* Assist clients with rehabilitation programmes e.g. following exercise programmes, activities of daily living, mobility and transfer practice. This is achieved through education and teaching which encourages and empowers clients and their carers to reach an optimum level of self-management.
* Observe client progress against planned goals and report changes of achievement or those that may indicate a deterioration in a client's condition/situation e.g. on a routine hand‑over meeting or urgent basis (immediately) depending on the priority.
* Carry out safe moving and handling practice, on a daily basis and requiring moderate effort, in accordance with planned programmes of care and Trust policies.
* Follow instructions given for fitting and operation of basic equipment once assessed as competent to do so, as required by qualified staff, and train clients, carers and other staff members in its safe use e.g. walking frame, toilet seat raise.
* When within the community setting (e.g. within a client's home) work as a lone practitioner with telephone support.
* Seek support and advice from more experienced colleagues when faced with matters outside of sphere of responsibility, knowledge or skills. (Cases will be reviewed during MDT meetings)
* Provide information to clients and carers on improving their health and well‑being and of any resources/services available to them.
