What youll be doing Delivering high-quality nursing care in patients own homes.
Supporting early hospital discharge and preventing avoidable readmissions.
Building meaningful clinical relationships with patients and MDT partners.
Completing IV therapy administration, wound care, venepuncture, and general nursing duties.
Working a mix of early and late shifts across a 7-day rota.
