Job Summary
To provide a high level of clinical expertise, to support and facilitate individualised patient centred care to patients with a life limiting illness and to provide care and support for relatives and carers in collaboration with the multi-disciplinary team.
Main duties of the job
* Demonstrates leadership through clinical expertise and specialist knowledge, delivering high standards of personalised care.
* Identifying needs through a holistic assessment and co-creating a care and support plan with people living with cancer and other life limiting illnesses.
* To independently manage own caseload of patients and associated responsibilities.
* To independently manage the role of duty nurse on rotation with colleagues.
* To assess, and triage new patient referrals when required, in the absence of triage specialist nurse.
* To assess, plan, implement and evaluate all aspects of the patients care.
* To provide symptom control, social, emotional, and spiritual care for the patient, family, and carers.
* To provide supportive care to patients, their families, and carers.
* To monitor patients on an ongoing basis, to evaluate outcomes and adjust care plans to ensure optimum co-ordination and delivery of care.
* To deliver high standards of care directly and assist in the management of junior staff.
* To develop and maintain effective relationships with all members of the multi-disciplinary team to ensure collaborative working for the benefit of patient care.
* To support the development and implementation of care pathways and patient information.
* To maintain accurate and up to date patient records.
* To ensure that the dignity, safety, and confidentiality of all patients is respected and that all patients receive the highest standards of care.

Person Specification
Essential
* Experience of nurse management
* Experience of working in a multidisciplinary team
* Knowledge of palliative care and updated clinical practice within palliative care
* Knowledge of national, regional, and local policy in relation to quality standards, targets, and improvements initiatives
* Experience in helping to develop clinical services
Desirable
* Multi agency working
* Experience in research and audit
* Proven ability to manage a budget

Qualifications
Essential
* Registered Nurse on the NMC Register
* Minimum of four years post registration experience or significant palliative or oncology experience
* Palliative care qualification
* Experience of ongoing professional development
* Mentorship qualification ENB 998, Mentorship, Practice Assessor/Supervisor
* Advanced Communication or equivalent experience
Desirable
* Masters Level Degree
* Management or Leadership certificate
* Teaching qualification
