Job Summary
We're looking for an experienced Advanced Clinical Practitioner to join our primary care team, delivering high-quality, person-centred care to residents in care homes and housebound patients. You'll assess, diagnose, prescribe, and manage frailty and long-term conditions, working closely with GPs, pharmacists, nurses, and care staff to improve outcomes and reduce hospital admissions.

Responsibilities
* Working with our existing clinicians to support the work of the GPs in the practice with care-home visits and ward-rounds, housebound patients and acute home visiting.
* Weekly care home rounds, liaising closely with care home staff, practice GPs and colleagues in support of their residents.
* Occasionally support the GPs by triaging and attending acute home visits in rotation with the established clinical team.
* Delivering support to our housebound patients to improve management of LTCs and help reduce hospital admissions.
* To deliver a high standard of patient care, using advanced autonomous clinical skills, and a broad in-depth theoretical knowledge base.
Person Specification
Experience
Essential:
* 5 years post registration experience.
* Recent experience within primary, community or emergency setting. Independent consulting.
* Ability to work under pressure with competing priorities
Desirable:
* Health-needs assessments including dementia
Qualifications
Essential:
* Registration NMC or Registration HCPC paramedic
* Evidence of ongoing professional development.
* Physical Assessment & Clinical Reasoning (PACR),
* Independent Prescriber
Knowledge & Skills
Essential:
* Clinical examination skills.
* Clinical practice skills.
* Management of patients with long-term conditions.
* Management of patients with complex needs.
* Accountability of own role.
* Clinical governance issues in primary care.
* Communication skills, both written and verbal
* Ability to assess a situation and get the support required
* Communication of difficult messages to patients and families
* Negotiation and conflict management skills
Desirable:
* Work within General Practice.
* Understanding of Quality Outcomes Framework Impact Investment Fund
