Liaison Mental Health Nurse/Occupational Therapist

This is an exciting opportunity for an experienced Mental Health Nurse or Occupational Therapist to develop knowledge and expertise within a specialist service, working with individuals who present with complex physical and psychological difficulties.

The team and wider department are well established and include a broad range of clinical expertise across Nursing, Occupational Therapy, Physiotherapy and Cognitive Behavioural Psychotherapy. The service is currently launching the TRACCS pathway in collaboration with the National Inpatient Centre for Psychological Medicine (NICPM). This includes a community-based model of care offering a combination of home visits, clinic-based appointments and virtual working, alongside established outpatient provision, and provides an excellent opportunity to build upon experience in pathway and service development.

We are looking for highly motivated clinicians to join our therapy team on a permanent basis. The post currently equates to 1.6 WTE, and we are therefore able to offer more than one post. Full‑time and part‑time working arrangements can be negotiated to support service need and individual preferences for the successful candidates.

We're looking to hold interviews for this post during w/c 9th February 2026.

You will work with a high degree of autonomy, assessing and delivering a range of psychosocial and psychological interventions for individuals referred with Persistent Physical Symptoms and/or Long‑Term Conditions. The service has strong links with Leeds Teaching Hospitals Trust and Primary Care, and you will be supported to develop and maintain effective interfaces with relevant partner services.

There is a strong emphasis on personal and professional development, with the team actively supporting individual development plans. You will also have opportunities to contribute to ongoing service development and pathway redesign as the TRACCS model continues to expand.

This is an excellent opportunity for an individual wishing to further develop skills in psychosocial interventions and therapeutic approaches. Experience in Liaison Psychiatry is desirable but not essential, as training and supervision will be provided for the right candidate.

The Leeds and York Partnership NHS Foundation Trust (LYPFT) is the main provider of Mental Health and Learning Disability services in Leeds. We also provide specialist inpatient services in York and some highly specialised services across the country. Applicants should be aware that individuals requiring a visa to work in the UK, the Trust only provides sponsorship for registered healthcare practitioner roles (this does not include Healthcare Support Workers). This is an essential requirement, and the Trust is unable to offer you a role if you do not meet Trust requirements for sponsorship and Home Office requirements for a visa.


Key Responsibilities

* Carry out biopsychosocial assessments for individuals referred to the service.
* Develop shared biopsychosocial formulations and agree evidence‑based treatment plans in collaboration with service users.
* Deliver a range of psychosocial interventions across outpatient and community settings, including home visits.
* Review patient progress continually throughout therapy and adapt plans in response to individual need.
* Work closely as part of a supportive multidisciplinary team, contributing to regular MDT meetings.
* Manage an autonomous caseload, with appropriate support and supervision, ensuring timely and regular contact.
* Contribute to the duty role and screening and triage of referrals into the service.
* Build and maintain effective links with colleagues across Liaison Psychiatry Services and including NICPM, Leeds Teaching Hospitals Trust and Primary Care.
* Provide clinical supervision where appropriate and support the development of others.
* Play an active role in service development, including leading or contributing to service improvement initiatives such as the TRACCS pathway.
