Post: Lead Practitioner
Service: South Lanarkshire Alcohol & Other Drugs Reachout Service
Based: Hamilton & Cambuslang
Hours: Full Time (37 hours)
Salary: £26,076 - £29,144 (£13.55 - £15.15 per hour)
Contract: Permanent


About the service

Turner Point Scotland South Lanarkshire Reachout Service offers a supportive team environment to provide intensive case management and post and pre-detox support. The service will operate an Assertive Outreach model across South Lanarkshire, with offices in Cambuslang, Hamilton, East Kilbride and Clydesdale. It delivers structured interventions including harm reduction advice, psychosocial support and community engagement, and supports people to prepare for detox, during detox and afterwards, in community and hospital settings.


Responsibilities

* Intensive Case Management and community engagement
* Assertive Outreach to bring the service to where people are
* Intensive person-centred approach and personalised support planning
* Structured psychosocial interventions (e.g., ABI’s, emotional support, motivational interviewing, crisis interventions, family support and onward referrals)
* Linkage with community resources and presence at community events to identify people needing alcohol-related support
* Support at alcohol-specific events and multiagency reviews
* Harm reduction activities including alcohol management plans, hydration and nutrition
* Use of alcohol screening tools to identify problematic or dependent use
* Collaboration with health professionals and partner services (e.g., CAReS)
* Support to engage with services and attend appointments when required
* Information and advice to help people make informed choices; pre- and post-detox support
* Preparation for detox and post-detox community support planning
* Planning for residential rehab pathways information/referral; family support
* Support to build recovery capital and linkage with recovery services
* Encouraging engagement with peer networks in the community
* Person-centred approach involving family, friends, health and social care professionals, advocacy services and staff


Requirements / Qualifications

* Experience within the Alcohol and Other Drugs sector
* Values of respect, compassion, inclusion and integrity
* Full driving licence and access to a vehicle is essential due to geographic area (mileage paid at 45p per mile)
* IT skills are required for all vacancies
* Ability to work across a 7-day week pattern (out of hours and weekends) as part of the service
* Willingness to register with the SSSC within 6 months of start date where applicable
