The Community Inpatient units are Geriatrician led, multidisciplinary team, with integrated pathways and collaborative working across acute and community services.

We are looking for a friendly, enthusiastic, and innovative individual who will work well within the team to meet service demands. We are seeking individuals with a keen interest in frailty and rehabilitation wanting to develop their skills and expertise.

This post is part of a wider community offer and there is opportunity for the post holder to see the other opportunities community working can offer.

Main duties of the job

1. York and Selby Community in-patient units

The post holder will work within the Selby and York Community in-patient units as part of the speciality of Community and Care of the Elderly Services.

The aim and main function of the community in-patient unit bed base is to provide short term rehabilitation, reablement, recovery and end of life care to the patients.

The post holder will work closely with the existing team of Advance Clinical Practitioners (ACP) to provide the medical clinical support services to ensure patients receive appropriate interventions, investigations, and treatment with minimal delay.

As part of the medical team you will be providing acute medical assessment and intervention whilst working closely with the wider MDT, supporting admission from the Hospital and the community and timely patient discharges from the in-patient units.

Detailed job description and main responsibilities

Generic responsibility and information.

The post holder will be a central member of a multidisciplinary team. The post holder will be supported in keeping themselves up to date with current knowledge & practice, expected to attend clinical meetings within the department & to participate in audit & “in-service” training of other healthcare professionals.

There are no on call requirement for these posts.

The post holder will may be required to deputise for absent colleagues from time to time and to perform additional duties in occasional emergencies and unforeseen circumstances.

It is essential that the post holder holds a full UK driving licence and has access to their own vehicle. Mileage will be reimbursed

The post is not a recognised training post, but would be ideal for those wishing to gain further experience in the specialty as the training, supervision and clinical experience will be similar to that of the training grade doctors and tailored to the postholder’s previous level of experience.

Candidates must have full GMC Registration, and a familiarity with issues of service organisation and development would be particularly advantageous.

The recruitment programme for this position will take place over one day.

qualifications and experience

Essential criteria

2. MBChB or equivalent medical qualification
3. Satisfactory completion of Foundation Year 1 and Foundation Year 2 post or equivalent.
4. GMC registration with license to practice
5. Current European Advance Life Support
6. A minimum of 6 months experience working in the Specialty of Medicine

Desirable criteria

7. 12 months experience working in the Elderly Medicine, Acute or General Medicine
8. Substantive NHS experience. (12 month continuous experience)
9. MD or equivalent higher degree or evidence of relevant research.
10. Research experience & peer-reviewed journal publications.

Knowledge and Skills

Essential criteria

11. Able to demonstrate a high level of clinical experience and skill.
12. Knowledge of good medical practice and GMC requirements.
13. Able to communicate effectively with multi-disciplinary team members, patients, relatives, GPs and other agencies.
14. Able to work effectively despite pressures, and to cope with stressful situations in a calm and reflective manner.
15. Understanding of current NHS issues.
16. Ability to work in partnership to deliver a patient centred service.
17. Commitment to Continuing Medical Education and the requirements of Clinical Governance and audit
18. Experience in teaching & supervising medical students and junior trainees.
19. Able to teach clinical skills
20. Written and spoken English adequate to enable effective communication about medical topics with patients & colleagues.

22. Able to establish good working relations with patients, carers and multidisciplinary colleagues
