The shift pattern for this post is: Five out of seven days per week. 08:30hrs - 16:30hrs for 4 days & 08:30hrs - 16:00hrs 1 day per week including weekends

This is an excellent opportunity for 1 WTE Care Home Advanced Nurse Practitioner (ANP) to join a highly motivated and dynamic team working across role and organisational boundaries. ANPs can work in or across all clinical settings, dependent on their area of expertise .Based within Renfrewshire HSCP, the ANP will be working with Care Homes to provide assessment and care for the patients registered within these practices. The ANP will complement core services provided by the HSCP and will also work alongside community nursing and rehabilitation services. The ANP will manage the complete clinical care for their patients utilizing an expert knowledge base and clinical competence in order to support multi-professional teams within NHS Greater Glasgow and Clyde, whilst ensuring a high standard of clinical care, in accordance with the Nursing and Midwifery Council and Health and Safety Legislation. As a clinical leader, the ANP has the freedom and authority to act autonomously in the assessment, diagnosis, treatment, including prescribing, of patients with multi-dimensional problems. This includes the authority to refer, admit and discharge within appropriate clinical areas.
The ANP will provide an accessible, appropriate, high quality, culturally sensitive and effective nursing service to support staff and residents in the care home setting, by working in partnership with service users, care providers and other agencies.
It is essential you are a Registered Nurse on the NMC Register, have completed the ANP training programme and have been signed off as an ANP and added to the SWISS register. Significant comprehensive post registration experience within a healthcare setting is essential. The shift pattern is hours, Monday to Friday excluding public holidays.
