The role will be Monday to Friday 8am-4pm/9am-5pm.
You will be a key member of the Intermediate Care Multi Disciplinary Team which provides a central point for the referral and provision of integrated community services, facilitating safe and timely care provision for our population. You will be expected to be kind and responsive, professional and informative and contribute to the quality of our services by demonstrating the behaviours detailed in our Values.
You will receive queries via telephone, email and face to face conversations and will resolve issues using your expertise of pathways and processes. To be successful in this role you need excellent time management skills, must be able to work on your own initiative, and be able to prioritise your own workload with minimal supervision.
The role is diverse, including taking referrals, being part of ICMDT discussion, liaising and arranging services. You will be working with a number of different professionals in a busy, supportive environment. Some knowledge of health and social care services would be beneficial as you will be dealing with diverse referrals and pathways, gathering information and coordinating responses.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
