Specialist Pharmacist - Stroke and Care of the Elderly

Barts Health Pharmacy Department has an exciting opportunity for a dynamic and enthusiastic pharmacist to join us as a Specialist Pharmacist for Stroke and Care of Elderly at the Royal London Hospital. Barts Health NHS Trust is one of the largest and busiest trusts in the UK, delivering care across multiple sites in East London and hosting the London Air Ambulance at The Royal London Hospital. The Royal London Hospital provides a full range of local and specialist services, including hyper-acute stroke care and complex medicine for older people, serving the population of East London.

This post offers the opportunity to work closely with the multi-disciplinary team to deliver and develop a high-quality clinical pharmacy service across Stroke and Care of the Elderly/Frailty pathways. You will provide senior clinical leadership, support medicines optimisation and safe transitions of care, and contribute to service development, governance and quality improvement.

You will join a supportive and integrated pharmacy team committed to delivering high-quality, patient-centred care. The successful candidate will have access to education and development opportunities, with support and mentorship from senior clinicians within Pharmacy and across other disciplines.


Main duties of the job

The post holder will carry out the full range of duties as the Specialist Pharmacist in Stroke and Care of the Elderly to deliver patient focused, quality care, based at the Royal London Hospital.

To promote the responsible, safe and cost-effective prescribing of medicines within medicine to optimise clinical outcomes, minimise adverse reactions and improve medicines safety. The post holder will be supporting the Lead Pharmacist in leading a team of staff in the delivery of high quality clinical pharmacy.

They will support the Lead Pharmacist in managing the professional, clinical and technical input of pharmacy into Medicine directorates at the Royal London Hospital and will help deliver the broader medicines optimisation and governance strategy across the Trust. These outcomes will be achieved primarily through leadership of clinical staff, and the publication and implementation of guidelines and procedures to encourage best practice.

Person Specification


Qualifications

* oMPharm or equivalent
* oRegistered as a pharmacist with the GPhC
* oDiploma in Clinical Pharmacy/ Pharmacy practice or equivalent.
* oIndependent prescribing qualification, or willing to undergo training to become a non-medical prescriber
* oAccredited with RPharmS Advanced Framework, or willing to works toward the accreditation
* oMSc in Clinical Pharmacy or equivalent.


Experience

* oExperience of providing a hospital-based service to medical and specialist ward at specialist pharmacist level or equivalent.
* oSignificant experience post registration in a hospital based clinical post.
* oExperience with stroke or care of elderly patients
* oExperience of providing evaluated information on drug expenditure.
* oThorough understanding of clinical governance and clinical risk management.
* oEvidence of basic teaching and tutoring in a hospital setting through training others e.g. pre- registration / basic grade Pharmacists.
* oExperience of successfully managing and supervising staff, performing appraisals and giving feedback.
* oSuccessful experience of managing and leading projects
* oExperience of using KPI and performance data to identify trends, propose actions to support medicines optimisation and service improvement.
* oExperience of providing clinical pharmacy input to palliative care / end of life care
* oExperience of developing business cases / service development
* oExperience of managing guidelines and PGDs relevant to the speciality
* oExperience of managing medication incidents


Knowledge

* oUnderstanding of GPhC Revalidation requirements
* oKnowledge of drug and therapeutics at Diploma level or equivalent.
* oKnowledge of medicines management systems at ward level
* oKnowledge of medicines management systems at ward level
* oUnderstanding of stock control systems
* oUnderstanding of the pharmaceutical care needs of Acute Medicine patients
* oAn awareness of current national standards, guidelines and service delivery issues relevant to Stroke and Care of Elderly patient
* oUnderstanding of stock control systems
* oKnowledge of Safe and Secure Handling of Medicines requirements
* oUnderstanding of the importance of adherence to SOPs and legislation
* oDemonstrates a good understanding of patient safety initiatives
* oKnowledge of interface issues relevant to the patient group.
* oKnowledge of risk management issues relevant to the patient group and root cause analysis.
* oKnowledge of rationing decisions in the NHS, Payment by Results (PbR) and commissioning.
