We are looking for a self-motivated, organized individual who is passionate about patient care and excited for the opportunity to independently manage their cases with confidence and accountability. As a Peritoneal Dialysis Registered Nurse (PD RN), you will provide education on peritoneal dialysis to patients and their care partners in a clinical setting, enabling them to dialyze from home.
You will be responsible for the complete ongoing care of each patient to ensure their treatment stays on track.
Dialysis experience is NOT required. Nephrology nursing offers a fresh perspective and skill set to those new to the field.
What you can expect:
1. Provide comprehensive patient care independently, applying core nursing skills to assess, diagnose, and implement complete care plans, considering the whole patient. As a case manager, you will track and organize patient care, witnessing the positive impact of your care.
2. Empower patients by teaching them to administer their own dialysis and take control of their health. Training takes time and effort, but the rewards are seeing patients transform from feeling sick and afraid to strong, independent, and active in their daily lives. Educating patients on dialysis options helps expand the program's reach to more who can benefit from PD.
3. Lead, coach, and guide with your nursing expertise. You will work independently while building strong relationships with patients, support systems, and care partners.
Schedule:
1. Typically, an 8-hour day, Monday through Friday, with weekends and holidays off.
2. Flexibility to manage your own cases and schedule.
3. Scheduled home visits and on-call shifts vary by program and are compensated accordingly.

Requirements:
* Current RN license in the state of practice
* Minimum of 12 months RN experience
* Valid driver’s license, reliable transportation, and insurance
* Associate Degree in Nursing (A.D.N.) required; B.S.N. or diploma may be substituted
* Basic computer skills, proficiency in MS Word and Outlook
Preferred (Nice-to-Have):
* Experience in chronic hemodialysis, acute dialysis, home hemodialysis, or peritoneal dialysis
* Background in training, teaching, or healthcare education
* Case management experience
* Familiarity with telehealth technologies
* Certifications: CNN, CDN, or CPDN

$46.00 - $63.00 per hour
