About the Role
In this role, you will:
* Undertake initial holistic nursing assessments in partnership with the client, their family and the care team.
* Develop individualized care plans in partnership with the client, their family and other healthcare professionals. Effectively communicate with staff on the clinical condition/diagnosis and treatment/discharge plans of clients and ensure accurate records are maintained by staff.
* Identify and manage risks as part of care provision and lead the investigation and resolution of clinical incidents, ensuring timely and thorough documentation.
* Support staff by delivering client‑specific clinical training, mentorship and supervision by providing feedback, conducting performance reviews, and supporting professional development.

Experience and Qualifications We’re Looking For
* A learning disabilities nurse with an active NMC pin
* Experience leading community teams to support individuals with learning disabilities, autism and mental health needs
* A valid drivers license and access to a vehicle
