Work base: Carers Trust Hillingdon Offices, 126, High Street, UB8 1JT

Hours: 0.78 fte/28 hours per week - negotiable


Purpose of role

* With support from the Chief Executive and Senior Managers, to conduct our Young Carer assessment function, as per the requirements of the London Borough of Hillingdon contract.
* To conduct initial assessments of new young people joining the service using a format as pre‑agreed with commissioners, and ensure comprehensive GDPR‑compliant records are entered and maintained on the in‑house Client Record Management System (CRM), Charity Log.
* Work with the wider team to ensure that this is a starting point and, as relationships develop, new information is captured about the lives and needs of the young people we support and their families, and we agree target outcomes for each young carer.
* From initial assessment, develop a proposed ‘pathway’ of support across the new structures i.e., the three support teams: Therapeutic Care & Outreach, Social Support and Supported Transition, and agree pathway with members of the relevant teams at regular case conferences.
1. To undertake all assessments for new young carers referred to the service – and re‑assess existing clients if a need is identified by the team.
2. To ensure that new information is captured as relationships with new clients develop and the team supporting them record this appropriately.
3. Home/School visit to conduct impact assessment, liaising with family to develop pathway.
4. From assessment, draft an initial support ‘pathway’, which best meets identified needs. Those experiencing the most negative impacts from their caring roles will need more extensive input from multiple parts of the team, whereas others might just need access to Club and social support. External referrals to appropriate services should also be considered.
5. Complex cases will need to be discussed at regular case conferences across multiple parts of the team. The postholder will agree these in advance with the wider team and convene meetings, ensuring that: a) young people are supported by all relevant services; but, b) there is movement across services and dependencies are not created.
6. To generate reports using Charity Log, and to use this data, other relevant information and case studies to complete monitoring reports for funding bodies.
7. To work flexibly as the demands of the role dictate and to be prepared to adapt to unforeseen challenges or changes.
8. To take part in regular team meetings, supervision and annual appraisals with line manager.
9. To take responsibility for own professional development and identify and attend regular relevant training courses and conferences.
10. To work flexibly, including occasional weekend and evening work, for which time off in lieu will be given.
11. To carry out the above tasks in accordance with the policies and procedures of Carers Trust Hillingdon & Ealing, including Equal Opportunities, Safeguarding, Confidentiality, Information Governance and Health and Safety Policies.

The above list of responsibilities is not exhaustive. The post-holder will undertake such duties that may be required from time‑to‑time that are consistent with the role and the needs of the organisation.

Due to the intense nature of the role, the postholder will have access to monthly group reflection sessions to share stresses and the Employee Assistance Programme (EAP) via Bright HR.

It is recognised that this is a new role and some work might need to be allocated to the wider team, so a regular review in the first 12 months should be built in.
