Job Overview
We have a permanent, full‑time (37.5 hour) opportunity for a new member to join our multidisciplinary Stroke Early Support Discharge team as a Band 3 Rehabilitation Assistant. This role will support patients following a stroke within the community, meeting their physical, communication, cognitive and functional needs.
Main Duties
* Provide stroke specialist rehabilitation in the community.
* Work as part of the multidisciplinary team under the direction of registered staff in a variety of settings, including patients’ homes.
* Deliver indirect tasks that support patient care and therapy needs, such as record keeping and equipment management.
* Include weekend working as required.
* Hold a full UK driving licence and access a vehicle insured for business use for independent travel across the work locality.

Person Specification
Essential Qualifications
* GCSE A‑D or grade 3‑9 in English and Maths or equivalent.
* NVQ Level 2/3 in care/rehabilitation or equivalent.
* Skills to manage challenging situations.
* Good organisational skills and decision‑making ability.
Essential Experience
* Experience working in a healthcare or rehabilitation setting.
* Experience working independently with people with physical and/or emotional needs arising from a long‑term health condition.
* Experience performing administrative duties.
Desirable Experience
* Previous community experience.
* Experience of being a generic worker within a multidisciplinary team.
* Experience as a Therapy or Nursing Assistant/rehab technician.
* Experience working with people with impaired ability to communicate.
* Experience working in the NHS.
Essential Skills, Abilities & Knowledge
* Full and valid driving licence.
* Good communication and organisational skills.
* Excellent IT skills, confident use of Microsoft Word, Excel, Outlook and PowerPoint.
* Ability to deal with distressing and emotionally difficult situations.
* Observation and reporting skills.
* Understanding of patient rehabilitation needs.
* Ability to maintain accurate, up‑to‑date records.
Desirable Skills
* Knowledge of other multidisciplinary team roles.
* Knowledge of the Mental Capacity Act (2005).
* Ability to use SystmOne.
* Experience with electronic patient/service‑user record systems.
* Ability to evaluate data and produce reports using Microsoft Excel and Word.

* Capability to manoeuvre patients using moving and handling aids.
* Full and valid driving licence (repeated as a key requirement).
* Proficient written and spoken English to enable full role performance.
