Job Description:
OneMK PCN is looking for an experienced Advanced Nurse / Clinical Practitioner to join our multi-disciplinary team, to help us transform and modernise pathways of care, enabling safe and effective sharing of skills across professional boundaries. We deliver patient care from 3 surgeries: Whitehouse, Neath Hill, and Brooklands.
The Advanced Nurse/Clinical Practitioner will make autonomous decisions in assessment, diagnosis, and treatment of patients. You will have primary care experience and high standards of patient care. The role includes clinical governance responsibilities and provides clinical leadership to ensure safe, effective, and compliant service delivery.
You will work with a team including GPs, nurses, wellbeing advisors, health coaches, physician associates, and more. The ideal candidate will be flexible, eager to learn new healthcare delivery methods, and committed to professional development.
Responsibilities include:
* Registration with NMC/HCPC/GPhC
* Master's in Advanced Practice or equivalent
* Certified Independent Prescriber
* Experience in primary care and minor illness management
* Managing care episodes within a multidisciplinary team
* Strong communication, patience, and leadership skills

Locations:
Brooklands Health Centre: Located in Broughton, with good transport links, close to bus stops and the M1.
Whitehouse Surgery: A state-of-the-art health centre serving Milton Keynes residents, with comprehensive staff and support services.
