Overview
The role is a 12‑month fixed term contract discharge planning specialist. The post holder will demonstrate expert knowledge of discharge options and services, support patients, families, carers and care workers in communicating the possible plan, estimated date of discharge, managing expectations and concerns. The post holder will aim for a safe and sustainable discharge by managing risks to reduce readmission.
Key responsibilities include contributing to multidisciplinary team meetings, ensuring patients’ wishes are heard, promoting advanced care planning, identifying social care needs, acting as a link between wards and community teams, completing specialist discharge planning, liaising with discharge planning nurse and working closely with healthcare professionals, clinicians, social work teams, family members, personal care services and multi‑disciplinary team members.
Responsibilities
* Observe and maintain strict confidentiality in line with the Data Protection Act.
* Ensure any data taken, shared or transferred electronically adheres to CPFT information governance and information security policies.
* Adhere to CPFT risk assessment and risk management processes.
* Adhere to infection control policies and procedures.
* Undertake mandatory training and any other training relevant to the role.
* Participate in clinical and other audits as required.
* Provide patients and their families/carers with information on standards expected from the team.
* Provide discharge planning training programmes and oversee issues across the Trust to ensure professional needs are met and trust discharge policies are adhered to.
* Create discharge education packages appropriate to different grades and disciplines across different settings.
* Identify, monitor, investigate and record reasons for delayed discharges and review ways of working with multidisciplinary teams to reduce these delays.
