What the Job Involves As a Social Work Assistant, you will be responsible for arranging and monitoring services in accordance with individual care plans. You will liaise with voluntary and statutory agencies, provide advice and practical help, and complete annual care plan reviews. You participate actively in team meetings and duty rotas and ensure that all duties align with Council Policies and Procedures. The role might also require you to work from various locations as necessary.
