Overview

The post holder will be a Mental Health Nurse or Mental Health Social Worker within the specialist Pathfinder partnership with survivors of sexual abuse and violence. The post is primarily identification of potential survivors who may benefit from Pathfinder support, including liaising with ISVAs and counsellors and other services, and providing support to members of the Multi-Disciplinary Team in relation to risk management and safeguarding issues. The post holder will deliver clinical appointments within a range of venues including GP practices and partner organisations across Greater Manchester, or in a home setting where clients\' needs necessitate this. Clinical appointments include initial face-to-face mental health, social care and risk assessments (including collecting standardised assessment measures) and case management of an assigned group of clients who require ongoing monitoring of their mental health and risk management, emotional and practical support; and the delivery of evidence-based interventions for depression, anxiety, and trauma. The role also includes identifying potential clients for the Pathfinder partnership via waiting list screening and engagement with ISVAs.


Responsibilities

* To present potential clients at consultation via the confidential consultation process and liaise with referrers and other service providers to ensure clients are treated in the most appropriate service for their needs.
* To provide a thorough bio-psycho-social assessment of the client (aged 16+), including risk assessment and a care plan.
* To deliver individualised interventions to the client based on the individual care plan.
* Liaison with all members of the Pathfinder partnership, HSP clinicians, Secondary Care Mental Health Services, and Voluntary/Statutory agencies across Greater Manchester.
* To manage own caseload by delivering brief interventions to the client, and participating in line management to plan endings and allocation of new clients.
* To deliver agreed clinical contacts as set by service.

All applicants external to Pennine Care NHS Foundation Trust will be required to provide HMRC employment history for the most recent three years. This information will be used to validate employment history and references. If you require sponsorship, we will assess your eligibility based on current government guidance. If the role is not eligible and you cannot demonstrate your right to work, your conditional offer may be withdrawn.

Once Checks Are Complete: the Recruitment Team will coordinate with you and the hiring manager to confirm a start date. You\'ll be booked onto a Trust Welcome Session and receive your Pennine Care NHS Terms and Conditions.
