* Deliver Tailored Support: Provide responsive, person-centred support to individuals with complex needs, including those experiencing or at risk of homelessness, and those presenting with challenging behaviour in relation to mental ill health and alcohol & substance misuse, helping them achieve positive outcomes and independence.
* Support with Housing Needs: Assist individuals in accessing, maintaining, and sustaining appropriate accommodation, including support with housing applications, tenancy management, liaising with landlords, and preventing homelessness.
* Foster Collaboration: Build open and trusting relationships with customers, colleagues, and partner agencies to ensure seamless and comprehensive support.
* Promote Participation: Encourage individuals to engage in social, educational and employment opportunities, preventing isolation and promoting self-determination.
* Manage Risks Effectively: Conduct thorough risk assessments and maintain professional boundaries, ensuring health, safety and compliance with safeguarding policies.
About The Candidate:
* Have an understanding of support needs and support planning.
* Have experience of risk assessing and risk management.
* Be a great communicator with the ability to build rapport.
* Have accurate record keeping and computer skills.

Essential:
* Experienced in supporting complex needs clients with substance misuse or facing the homeless pathways.
