Senior Community Learning Disability Nurse

An exciting and rewarding Band 6 Senior Learning Disability Nurse role within an established Integrated Team that works alongside an MDT including social workers, nursing, psychiatry, psychology, physiotherapy, occupational therapy, speech and language therapy and administrative staff.

Durham County Council is the lead authority for the Integrated Learning Disability Teams. The candidate will use the Durham County Council Azeus system as the primary recording system and will still have access to TEWV recording systems and NHS mail.

The service operates 9am to 5pm Monday to Friday and includes a nurse on call rota covering out of hours 8pm to 8am.

Candidates must have at least 18 months of post-qualifying experience and have completed the appropriate preceptorship or competency framework sign‑off.


Main duties of the job

Deliver high‑quality secondary care and enhance community services for adults with learning disability and challenging behaviours, using trauma‑informed care and a recovery‑based approach.

Support the MDT with assessments, treatment, monitoring and evaluation of outcomes, provide clinical supervision, advice and guidance to junior staff, students and colleagues, and maintain effective communication and documentation.

Travel independently to various locations within the Trust and on a regular basis. Applicants must have access to a suitable vehicle for business purposes. Adjustments can be considered in accordance with the Equality Act 2010.


Job responsibilities

Deliver a quality service that places the individual at the centre of their care, ensuring full involvement in assessment, planning, care and treatment. Maintain professional and legal accountability.

Provide assessment, advice and support to clients, carers and other professionals, deliver training, and manage a clinical caseload under the supervision of the Professional Lead.

Work independently and as part of a wider team, supporting and developing the team, and contribute to quality improvement activities.


Person specification – Qualifications and Experience

* Current professional registration with Nursing & Midwifery Council – RNMH, RNLD (or RNMD for CAMHS) or RGN where appropriate.
* Evidence of continuing professional development.
* Recognised practice supervisor and/or practice assessor, clinical educator or equivalent achieved within 6 months.
* Trained in clinical supervision and capable of providing clinical supervision, or willingness to work towards this within 6 months.
* 18 months of post‑qualifying experience including sign off of an appropriate preceptorship or competency framework.
* Experience in working collaboratively with service users and their families/carers.
* Experience providing clinical supervision to individuals or groups where appropriate.
* Experience supervising or assessing students on practice placement.
* Experience working in a multi-disciplinary team and in quality improvement activities.


Person specification – Knowledge

* Demonstrable knowledge to a post‑graduate level of evidence‑based practice in caring for patients with learning disability.
* Understanding of relevant legislation, including the Mental Health Act, Mental Capacity Act and safeguarding.
* Knowledge of clinical risk assessment and management, clinical governance, research and development methodology, psychological models of care and treatment.
* Working knowledge of clinical medicines management, including administration of injections.
