Are you a passionate and motivated nurse looking to develop your career in renal care? We are looking for an enthusiastic Band 6 Specialist Kidney Nurse to join our friendly and supportive advanced kidney care (low clearance) team. This is an exciting opportunity to work with patients with advanced kidney disease from North central London, supporting them through treatment choices, education, and long-term care. You will play a key role in delivering high-quality, patient-centred care.
Responsibilities
* Liaise with all key members of the multidisciplinary team to advise and support colleagues in the delivery of specialised patient care across professional boundaries.
* Undertake nurse-led clinics for patients with advanced kidney disease and be responsible for excellent standards of nursing care to patients through the planning, co-ordinating, delivering and evaluating nurse-led clinics.
* Work with the support of clinical nurse specialists, lead nurses and consultants to deliver specialist clinical care appropriate to the needs of the patient group and based on principles that are research based and which enhance the quality of patient care.
* Contribute to the facilitation of the planning, delivery, and evaluation of care pathways for renal patients.
* Drive and coordinate training and education with a particular focus on the promotion of home-based therapies for patients requiring renal replacement therapy.
* Promote and coordinate patient participation and peer support for patients within the service.
* Promote an innovative and progressive attitude to the continual improvement of patient care through reviewing the evidence base and embedding this in practice through engagement in research activities.
* Participate in the development of staff within the multidisciplinary team providing support, advice, formal and informal education.
* Assist in the research and audit process.
* Act as a visible role model.
* Work at all kidney care sites.
