Healthcare Assistant - End of Life Urgent Care Service (EOLUCS)
Salary: £21,965 Per Annum (£23,875 Full-Time Equivalent)
Hours: 34.5 Hours Per Week
An exciting opportunity has arisen for an enthusiastic, well-motivated individual who is interested in end-of-life care.
St Luke's is delivering on its vision of a ‘Hospice without Walls’ with the support of its End-of-Life Urgent Care Team. As a Healthcare Assistant for the End-of-Life Urgent Care Service you will assist in supporting community-based end of life (EOL) patients in meeting their preferred place of care/death and prevent inappropriate admission to secondary care.
Whilst this role can be busy and challenging, there is great reward in helping make a real difference at the end of a person’s life by caring for them in their own homes and preferred place of care/death.

Responsibilities
* Support community-based end-of-life (EOL) patients in meeting their preferred place of care/death and help prevent unnecessary admission to secondary care.
* Care for patients in their own homes and care settings in a compassionate and professional manner.
* Coordinate with the End-of-Life Urgent Care Team to ensure timely support and appropriate escalation when needed.
Qualifications / Requirements
* Enthusiastic, well-motivated individual with an interest in end-of-life care.
* Ability to work in a busy community setting and to communicate effectively with patients, families, and the multidisciplinary team.
* Willingness to participate in training and development related to end-of-life care.
