Rapid Transfer Service EOL Discharge Coordinator

The Rapid Transfer Service (RTS) are responsible for managing complex patient discharges from the acute hospital to community pathways to enable the patients care needs to be assessed outside of hospital. Thispost is to assess end of life patients in the acute hospital who wish to return to their own home or to a care home. The role will include working within the acute Trust to assess patients' needs for discharge.

The post holder will work as a key member of the RTS, whose aim is to ensure the timely, effective, appropriate and safe discharge of end of life patients requiring supportive discharge, from the acute into a community setting.

The RTS service is a 7-day service covering weekends and bank holidays on a rota basis. The RTS team has responsibility for complex discharges across all 3 acute sites in east Kent and occasionally cross site working will be needed to ensure service cover.

This post is not delivering direct patient care. However, a clinical nursing background is essential.


Main duties of the job

Role Specific Responsibilities

* Assessment, planning, implementation and evaluation of holistic care linked to end of life care needs
* Supporting in the supervision and development of staff.
* Actively working as a member of the integrated team to provide high quality care to service users.
* Undertaking a range of competent clinical and non-clinical interventions to support the outcomes of the service.
* Acting as lead and senior practitioner to support junior staff carry out assessment/reassessments of service users and/or pathways with complex needs across the relevant field.
* Supporting and making recommendations to senior managers in order to prioritise and focus efforts to drive forward and achieve business objectives, service improvement strategies and statutoryrequirements

Person Specification


Qualifications

* 1st Level registered nurse NMC registration
* Evidence of Specialist qualifications relevant to area of practice e.g. Mentorship
* Community experience or qualification.
* Experience of working within both acute and primary care.


Experience

* Experience in area of practice
* Evidence of recent study and continuing professional development.
* Experience of working within a Health &/or Social Care environment at Band 5 (or equivalent).
* Supporting others (mentor / educator / supervisor) experience
* Experience of leading and managing teams
* Experience with the use of Microsoft Office and a variety of IT packages (E-Pay / E-roster/ AT-P/ CIS


Special Knowledge and Expertise

* A good understanding of the importance of discharge planning.
* Evidence of continued relevant professional development and implementation in practice within the last 12 months
* Knowledge of National Polices and Acts relating to legislation to support safe and sustainable discharge.
* Previous EOL experience


Skills and Abilities

* Excellent and effective communication skills, written and oral.
* Ability to converse in a positive manner with all levels of staff, patients and relatives/carers.
* Resourceful, ability to work on own initiative and apply judgement.
* Ability to remain calm under pressure, and in stressful situations.
* Demonstrate understanding of issues related to confidentiality of data and patient information.
* Ability to act as the patient's advocate.
* Able to demonstrate a patient orientated approach to work.
* Ability to demonstrate personal resilience, especially under times of pressure
* Demonstrate an understanding of issues related to equal opportunities.
* Experienced and proficient in word processing, database and spreadsheet
