We are working with a client in the Liverpool area who is looking for a dedicated Reablement Assistant to join their Adult Social Care team. This is a great opportunity to support adults in regaining independence and confidence within their own homes. As a Reablement Assistant, you will support adults with day-to-day tasks, help them rebuild daily living skills, and encourage them to live as independently as possible. You will work as part of a multidisciplinary team to deliver personalised, person-centred support. Key Responsibilities Support individuals with their physical, social, and emotional needs as outlined in their reablement plan. Assist with daily tasks such as personal care, meal preparation, and mobility. Help service users develop or regain independent living skills. Work alongside health and social care staff to reduce risks and maintain safety at home. Follow and contribute to person-centred support plans, assessments, and reviews. Maintain accurate records and report any changes to your Line Manager. Support individuals with activities and community participation. Follow all Council policies, including equality, dignity, and anti-discriminatory practice.
