Crisis practitioner - crisis assessment and intensive home treatment team

Duration, Location, and Working Pattern

* Location: Charleston Centre
* Contract: Permanent
* Working hours: Full time, 36 hours per week
* Shift pattern: Monday to Friday 09:00 to 20:00; Saturday and Sunday 09:00 to 17:00


Key Responsibilities

* The assessment, planning, intervention and evaluation of client care and practice.
* Support the effective management of clinical, practice and environmental risks on a day-to-day basis.
* Identify and respond to the health promotion needs of service users and carers.
* Identify and respond to needs related to gender health, domestic abuse and child protection.
* Maintain accurate records of care and practice.
* Develop therapeutic relationships with service users and carers to ensure care needs are identified and met within a participative framework that recognises equality, dignity and respect, taking into account spiritual and cultural diversity.
* Ensure personal practice is in line with the Mental Health (Care and Treatment) (Scotland) Act 2003 and the Patient Focus Public Involvement and Fair for All guidelines.
* Ensure personal practice aligns with national and local policies, standards, guidelines and good practice statements.
* Conduct critical analysis of personal practice and demonstrate a continuing commitment to professional development with the line manager.
* Provide appraisal, clinical supervision and support for junior team members through regular caseload management systems.
* Assist Senior Crisis Practitioners and Team Leader in the rapid deployment of crisis resources in response to service user need.
* Interface between the Community Mental Health Team and inpatient services, and other mental health network services and agencies.
* Prioritise workload and utilise resources to ensure all service user needs are met, accountable to the IHTT Team Leader.
* Participate in discharge planning and immediate follow-up from acute inpatient care, including assessment while on pass from acute care.


Qualifications, Training, and Experience

* Completion of training as a first-level registered nurse in mental health, or an occupational therapist registered with the Health Professions Council, or a qualified social worker registered with the Scottish Social Services Council.
* Post-registration experience in the mental health sector, preferably community.
* Evidence of continual professional development.
* Effective communication skills.
* Ability to work without direct supervision.
* IT literacy, basic level of Microsoft Word, Excel and Outlook.
* Current valid registration with the Nursing and Midwifery Council and a live PIN with the NMC UK.
