Description Integrated Care Coordinator Service Lead | 9676 | Secondment opportunity/Fixed term contract for 18 months | 37 hours per week | £43,822 to £48,227 per annum | Grade K | Norwich We are looking for a motivated Service Lead for Integrated Care Coordinators (ICC) to support the delivery and development of local services, strengthen partnerships across health, social care and community services, and guide ICCs to provide high-quality, person-centred care. Integrated Care Coordinators (ICCs) play a key role in the NHS Neighbourhood Health model by joining up care and improving communication across GP practice, NHS organisations, mental health, social care and community services. Working across Central Norfolk (North Norfolk, South Norfolk, and Norwich), ICCs follow shared standards while adapting to the needs of the communities they serve. ICCs identify people who need extra support, coordinate care from multiple professionals, and ensure individuals and their carers receive timely, seamless, and person-focused care. By helping teams communicate and work together, ICCs support people to stay independent, reduce unnecessary hospital admissions, and help build healthier, stronger communities. In this team leadership role, you will: Support the delivery and development of ICC services, helping the team to work effectively and consistently. Ensure processes and standards are followed across ICC teams, including referral handling, communication, and data recording. Make day-to-day operational decisions to support smooth running of the service. Work with colleagues across health, social care, and community services to provide guidance, training, and ongoing support for ICCs. Represent the ICC team in local meetings, sharing updates and demonstrating the impact of their work. Provide supportive and inclusive leadership to ICCs and Team Leaders across Central Norfolk. Help manage staffing and resources to ensure a consistent, effective service. You will be a supportive leader who can motivate and guide a team, build good relationships across services, and help solve problems as they arise. You will bring experience working in health, social care, or a related field, bring excellent communication, leadership and problem-solving skills. Have experience managing staff, navigating competing priorities and delivering service improvements. Confidence in using digital systems and a willingness to develop your skills further are important.
