We are looking to recruit a highly motivated, innovative and enthusiastic Primary Care Service Lead to join our team at HMP/YOI Bronzefield. By joining our Prisons Health Care Team, you will look after the healthcare needs of patients residing in secure environments and have a real impact on promoting health and well‑being. Main duties of the job
* Overall responsibility for line‑management, clinical, operational and performance functions; ensuring effective and efficient resource utilisation, awareness of financial impact, delivery of evidence‑based patient care and a safe service.
* With the Head of Healthcare, provide clear, visible leadership, vision and clinical expertise to enhance the service user experience.
* Implement contracted service development and improvements, day‑to‑day clinical service performance, workforce and budget management, professional clinical leadership and clinical governance, incorporating service quality, patient/offender safety and care standards.
* Monitor and report on all relevant contractual targets including the DoH’s Prison Health Performance & Quality Indicators.
* Act as the senior nurse manager contact for primary care on all managerial and clinical matters.
* Attend Trust and Prison‑led meetings as required; effectively integrate primary care services with all other aspects of the prison health service and the prison’s core day.
* Operate flexibly within a challenging healthcare environment involving multiple providers, while being an ambassador for Healthcare and CNWL.
* Support the team in creating effective pathways of care for patients served.
* Deputise for the Head of Healthcare and oversee peer healthcare service manager areas as required.
* Work within a multi‑disciplinary service team approach.

Detailed Job Description And Main Responsibilities
* Deliver effective leadership and direction to all staff within the Primary Care Team.
* Ensure the service is cost‑effective, well‑organised, evidence‑based and service‑user focused.
* Ensure all targets and performance indicators agreed with the Head of Healthcare are met; communicate risks to the Regional Operations Manager and Business and Performance Lead as soon as identified.
* Implement service changes when required, ensuring effective implementation with measurable outcomes and agreed timescales.
* Advance innovative ideas and strategies and provide vision and inspiration to enable staff to deliver care more effectively.
* Provide clinical leadership to multidisciplinary teams across the wider healthcare service.
* Provide specialist primary care nursing advice to the GPs and other colleagues in the prison.
* Lead the realisation of everyday practice within a Clinical Governance framework, including setting the developmental agenda, incident and complaint management.
* Ensure effective clinical management of patients in in‑patients, segregation, wings/houseblocks and any other area of the prison.
* Conduct death-in‑custody reviews and action plans, determining standards, auditing performance against those standards, managing clinical risk, identifying best practice and addressing remedial actions as required.
* Lead the development of nursing practice in primary care and chronic disease management.
* Ensure the clinical environment meets acceptable levels and monitor cleanliness, safety and infection control.
* Act as a suitable role model for your grade of employment.
* Ensure all relevant documentation is completed in accordance with Nursing and Midwifery Council and CNWL Trust policy.
* Maintain standards of Caldicott and the Data Protection Act for patient confidentiality at all times.
* Represent the prison healthcare service and CNWL’s Health & Justice Service at appropriate meetings as required.
* Be compliant with the Modern Matrons Charter.
* Ensure services are compliant with the Care Quality Commission’s Provider Compliance Assessments for Quality and Safety.
* Ensure that working conditions and use of resources satisfy current legislation and establishment guidelines.
* Implement a culture that focuses on improving outcomes across the care pathway for clients.
* Work in partnership with the Prison and other healthcare providers to ensure clear and responsive operational policies, procedures and protocols.
* Take part in the ACCT process and other risk management processes.
* Ensure service users’ health needs are met through equity, fairness, respect and entitlement.
* Line‑manage the Primary Care Clinical Team leads and other nurses in primary care, ensuring monthly supervision and annual appraisal in line with Trust policy.
* Ensure the entire primary care workforce, including healthcare assistants and administrators, receive monthly supervision and an annual appraisal.
* Lead recruitment of vacant clinical posts in primary care for the Health & Justice Directorate as requested by the Head of Healthcare.
* Ensure all primary care nursing staff maintain professional registration and personal development by completing mandatory training and pursuing special interests (e.g., phlebotomy, wound management).
* Take the lead on implementing care quality standards, keeping updated with national policy and legislation, attending relevant Trust meetings, and driving improvement programmes across the service.
* Provide cover for the Head of Healthcare as required.

Education/Qualifications
* Professional Registration RGN – Nursing and Midwifery Council
* Evidence of continued professional development (e.g., ANP)
Experience
* Experience of senior nursing positions in healthcare services for adults.
* Experience of high‑level leadership qualities; multi‑professional and multi‑agency working.
* Preparation of reports and presentations to internal and external agencies.
* Experience of Safeguarding legislation.
* Experience of delivering nurse/AHP‑led clinics and leading on the development of these clinics.
