Seven Social Care is searching for a Social Work Assistant, you will be working closely with social workers to provide support to vulnerable individuals and families in Ollerton.
You will assist in conducting assessments of clients’ needs, and with planning and implementing care plans. You will also support with the delivery of interventions and services that aim to improve their overall well-being.
The rate of pay for this role is £14.70 P/H.
Key Responsibilities:
– Supporting Social Workers in conducting assessments of clients’ needs, strengths and risks
– Assisting with the preparation and implementation of care plans
– Providing emotional support to clients and their families
– Coordinating and liaising with other agencies and professionals to provide holistic support to clients
– Providing practical support and assistance to clients with daily living activities
– Maintaining accurate and up-to-date records for clients
– Attending case conferences, review meetings and other professional meetings as required
– Adhering to policies and guidelines relating to safeguarding, confidentiality and data protection.
Requirements:
– Relevant qualification in Social Work, Health and Social Care or equivalent.
– Experience of working with vulnerable individuals and families in the community
– Excellent communication and interpersonal skills
– Ability to work effectively as part of a multi-disciplinary team
– Good organizational and time management skills
– Strong problem-solving skills
– Knowledge of key issues related to safeguarding, confidentiality and data protection.
