Specialist Community Public Health Practitioner – Health Visitor

Whatever your experiencewe will ensure you receiveindividualised and effectivesupport from both your health visiting line manager and colleagues within your team.
You will work within theBracknellarea to deliver core and targeted services to children aged 0-5 and their families.
For this role you will require excellent communication and interpersonal skills as well asa passion for working with children and families.
Team working, flexibility and the ability to work in partnership with other teams and agencies is essential. You will collaborate with Children's Centre's in the implementation of the Healthy Child programmeunderpinned by the Solihull Approach model of working.
Ifsuccessful,your base will beat one of the local Children'sCentres;however,your role will require you to visit homes across the Bracknell locality footprint (mileage will be compensated).
Main duties of the job
Develop and implement outcome focused care plans and evaluate care given, in partnership with clients.
Provide support to families using the Solihull approach model in behaviour management, parentingskillsand delivering programmes of care to vulnerable families.
Develop andfacilitateHealth Visitingdrop-inclinics, health promotionactivitiesand groups.

Education/Qualifications/Training
* Registered Public Health Nurse (Health Visiting) active on the NMC register
* Non-medical prescriber
* S.L.I.C.E/ENB 998 or equivalent
Continuous Professional Development
* Evidence of continual professional development
Previous Experience
* Experience in management of cases where a child has a protection plan
* Teaching experience
Knowledge, Skills & Abilities
* High standard of clinical work
* Basic IT skills/prepared to undertake training

Additional Requirements
* Car Driver who holds full driving licence
