Overview
We are hiring Clinical Support Workers within the state-of-the-art Willow Unit at Norwich Community Hospital. The unit supports 48 patients in a supportive environment, helping them achieve step-by-step improvements, independence, and readiness to return to the community.
Key Responsibilities
* Work as part of a multidisciplinary team to deliver care in a reablement setting.
* Support an environment where all team members uphold a reablement ethos and a positive risk-taking approach.
* Facilitate patients' independence with washing/dressing, nutrition, and daily activities.
* Ensure that highest standards of care are met within a quality framework.
* Support and deliver innovation within discharge practice and pathways.
* Coordinate with system partners, including voluntary sector, to ensure seamless transfer of care.
* Deliver and co‑coordinate care to a named group of patients.
* Work proactively to achieve unit and trust goals and objectives.
* Promote recognised policies, protocols, and guidelines.
* Carry out the reablement competency framework to support patients' stated goals.
* Report concerns or changes in clinical condition to registered staff.
* Support patients' emotional wellbeing by promoting independence and positive conversations.
* Accurately document all care delivered electronically, maintaining confidentiality.
* Respond appropriately to clinical emergencies and follow procedures.
* Adhere to trust policies, procedures, and nursing guidelines.
* Make patients welcome to the clinical area and participate in admission under supervision of registered staff.
* Provide high standard of individualised personal care (e.g., washing/dressing, nutrition) with a reablement ethos.
* Keep the unit clean, tidy, and safe, following infection control procedures.
* Chaperone/escort patients to other clinical departments or NHS units under direction of registered nurse.
* Support relatives under guidance of MDT.
* Assist with the discharge process for patients by the MDT.
