We are looking for a Support Workers to join our new Community Based Support Service in Wokingham. Our community based service works with Care Leavers aged 18+. As a Support Worker you will have a flexible, person-centred, approach to ensuring young people can successfully sustain their accommodation, develop independence skills, and transition into adulthood with resilience, safety, and stability. What you'll do: 1. Provide weekday support to help young people settle into their home and local area. 2. Assist with daily tasks such as appointments, paperwork, emails, and travel. 3. Support with tenancy issues, landlord communication, and reporting maintenance. 4. Help young people access local services (GPs, dentists, gyms) and apply for Universal Credit.
