Job Summary:
Work as Care Navigator to effectively manage patient flow in a busy Urgent Treatment Centre. Working as part of a team consisting of Doctors, Nurses, and Healthcare Assistants in a demanding environment providing a professional and courteous response to patient requirements, ensuring safe and seamless transfers between the different areas of the urgent and emergency care service with adherence to SELDOC values and procedures. The role involves general reception and clerical duties, including data inputting on to SELDOC Adastra within the Urgent Treatment Centre to ensure the smooth running of the unit. Key responsibilities will include providing information to help patients access appropriate health services, assisting diverse communities in finding the right support and supporting staff in collecting patient feedback to enhance care quality.
Key Responsibilities:
· To register the patient onto Adastra when they arrive
· To help unregistered patients to register with a GP practice
· To provide patients with information as required
· To assist with monitoring the waiting room for patient safety
· Adhere to policies on the need for secure management of patient identifiable data, respecting the right of patient confidentiality where applicable
· Answer and manage any enquiries as and when necessary
· Attend team meetings and undertake any training as will be required
· Be an effective communicator with all staff and Patients – verbal, non-verbal, written and electronic in order to be effective.
· Directing calls to appropriate extensions, dealing with telephone enquiries and recording messages when required
· Ensure all patients are treated with dignity and respect at all times
· Explain the complaints procedure should the patient wish to complain
· Follow the agreed guidelines to support successful patient registration with a GP practice of the patients choosing and help the patient access the right place for their care
· Highlight and follow-up any concerns/issues to the appropriate manager
· Monitor the waiting room consistently and alert the Shift Lead or clinical member of staff if a patient is visibly deteriorating
· Review and update reception and service information as required for the benefit of the public and staff
· Sort and re-direct incoming post and sort out outgoing post on a daily basis
· Support patients in completing friend and family tests and online feedback on their experience, including the implementation of patient surveys and audits/collation of data
· Act responsibly in respect of own and colleagues’ health and safety, welfare, security and follows all safe work practices in line with Health and Safety
· Assist in keeping the reception/waiting room area tidy at the end of each day
· Ensure patients feel informed, supported and listened to so that they can make meaningful decisions and choices about their care
· Receive all visitors of the urgent care centre with courtesy and efficiency, directing them as appropriate function and acting as a vocal point for enquiries. To book patients in as required in the appropriate IT system
· Work alongside and support the Admin Manager to help implement new systems and procedures within the clinic/department
· Supporting the Admin Manager with any administration tasks

Hourly rate from £13.85 - (normal working hours) £17.92 (bank holidays)
