C4 Care is a growing complex home care provider, delivering high-quality, person-centred support to individuals with complex needs. We’re now looking for 3 Care Coordinators to be the central link between our clients, care staff, and management team.
The Role:
As Care Coordinator, you’ll work closely with the Registered Manager to ensure care is delivered safely, consistently, and to the highest standards. You’ll be responsible for coordinating care packages, supporting and supervising care staff, and ensuring continuity of care for our clients.
No two days are the same, but every day is meaningful.
What We’re Looking For:
You’ll be someone who is organised, compassionate, and confident juggling multiple priorities in a fast-paced home care environment.
Essential requirements:
- Minimum 1 year’s experience in a similar care coordination or senior care role
- Full UK driving licence and access to a vehicle
- Experience within domiciliary or complex home care
- Strong understanding of CQC standards and compliance
Desirable:
- Level 3 in Health & Social Care (or working towards it)
- Experience supervising or supporting care staff
- Ambition to progress into management in the future
