Substance Use Caseworker (Children and Young Persons and Transitions) Cwm Taf Morgannwg Job Description Role Substance Use Caseworker (Children and Young Persons and Transitions) Service Cwm Taf Morgannwg Location: Flexibility to work across all areas of Cwm Taf Morgannwg, where appropriate is essential. Base: To be agreed on appointment Hours: 37 hours per week. Some evening and weekend work may be required to meet the need of the service. Salary: £26,790 - £28,880 per annum Reports to: CTM Team Leader Team Members Reporting to Job Holder: No direct reports. Contract: Permanent, in line with funding. ​ THE SERVICE ​ The aims of CTM Substance Use Service are to engage people who use drugs or alcohol, and or their concerned others, to access support to make positive life changes. This includes early intervention, engaging and assessing service users, needle exchange, brief interventions, outreach, training, case management, health promotion and harm reduction initiatives and recovery and aftercare services. Barod services are provided in a non-judgmental, non-discriminatory, and professional manner and in a way that demonstrates courtesy and respect for individuals using substances, and sensitivity to their unique personal situations and experiences. ​ ROLE PURPOSE The Children and Young Person and Transition Worker (CYPT) offer drug and alcohol support services for children, young people aged up to 25 and young concerned others who are experiencing problems in relation to drug and/or alcohol use. This post will also work alongside other agencies to offer drug and alcohol support services in the community. This will include education, ensuring young people are aware of the risks associated with substance use. We will also ensure that young people in our community have early access to structured support and intervention through community based awareness raising and education sessions in youth settings. ​ ​ MAIN DUTIES AND RESPONSIBILITIES ​ AIMS To work alongside colleagues to deliver substance use interventions to CYPT people identified as having substance use needs. T o collaborate with other agencies, including Cwm Taf Morgannwg UHB, third sector agencies and local authority teams, to provide a holistic integrated substance use service. RESPONSIBILITIES Providing face to face therapeutic intervention, care coordination and support to children and young people seeking help to tackle their drug and alcohol use. To use prevention and outreach activities to engage proactively with CYPT and potential high risk groups to allow early identification of substance use issues and prompt interventions as appropriate. Provide targeted outreach services to individuals who are identified as 'underrepresented or under reported' to encourage engagement in the CYPT treatment services. Use a range of evidence based age appropriate interventions and methods that have been agreed for use within the service. Conducting risk and comprehensive assessments in order to assess individual needs, identify goals, prepare a support package and to review outcomes on a regular basis. Providing crisis intervention where necessary and appropriate. To promote the single point of access entry point to CYPT and professionals. To work with CYPT in a way that promotes the Trauma Recovery Model Inspire and enthuse CYPT to maintain positive lifestyle changes. Signposting CYPT on to other relevant agencies. Empowering individuals and enable them to help themselves through informed choice and decision making. Ensuring access to and maintenance of appropriate support networks and services for young people. Improving and supporting young people's motivation for change. Supporting CYPT to maintain or build positive social networks. Ensuring all CYPT are contacted if they disengage with the service. Signposting identified populations affected by drug and alcohol use to appropriate services where they fall outside of the remit of the specification. Involving families and carers in the treatment of young people where approrpriate. Deliver mobile needle exchange services where relevant. Deliver mobile C-Card services. Deliver harm reduction initiatives including Naloxone and Nyxoid training, take home kits and Dried Blood Spot testing where age appriopriate. To ensure consultation and participation with young people in all aspects of service delivery. Maintaining good working relationships with key partners across all Tiers in particular CAMHS, Integrated Family Support Services, Social Services, Family Services and YJS. Liaising with referring agencies regarding individual assessments, needs and progress. Accessing other necessary services and resources identified in conjunction with individual client, through referral, negotiation and advocacy where appropriate. Deliver talks, workshops, presentations, campaigns and events and provide training where appropriate. Enter data onto the case management system, Palbase and use the system effectively to inform work with service users and concerned others. Provide training or talks to internal staff or external organisations as required. Other duties Contribute to the development and review of organisational policies and procedures as and when appropriate. ​ Positively promote and represent Barod in the public / external arena including engaging with media. To adhere to all Barod policies and procedures To ensure the undertaking of appropriate monitoring, review and evaluation procedures To undertake appropriate administrative tasks To co-operate with requests from Barod management and administration To take up Barod’s arrangements for support and supervision and participate in all relevant internal meetings Retain and maintain all information (paper and electronic) in line with service level agreed guidelines and statutory protocols, such as GDPR Undertake other reasonable duties as requested, but which may not be specified within the job description ​ PERSON SPECIFICATION Ideally all these requirements must be met by the candidate on appointment.However, in certain circumstances a candidate may be appointed who does not meet a particular requirement. This is providing that the shortfall can be made good in a reasonable time, and the candidate brings other skills, knowledge or experience which are valuable to the role and the organisation. ​ ​ Essential Desirable Qualifications and training ​ ​ Level 3 in Advice and Guidance (or equivalent) or willingness to work towards. Experience in any of the following areas: Safeguarding Boundaries Substance Awareness Harm Reduction Safer Injecting Psychosocial Interventions Youth Work Role-related knowledge, skills & experience ​ ​ Experience of working with children and young people. Ability to work effectively with people regardless of their ethnic, cultural, social backgrounds, their gender, age, religious belief, disability and sexual orientation. Proven ability to network and build professional relationships. The ability to compile, maintain and complete accurate written records, including data capture and the use of case management systems. Knowledge of substance use treatment and care pathways. Knowledge of the Welsh Government's drug strategy and Recovery Framework. Knowledge of substance use including harm reduction advice, information and interventions. Experience of providing early identification and preventative interventions. Experience of facilitating group work to children and young people. Knowledge of safeguarding procedures. Experience of community engagement ​ Role specific requirements / Interpersonal Skills ​ ​ Flexibility to work at multiple locations and flexible working hours outside the normal working week Ability to establish and maintain clear boundaries in respect of personal and professional responsibility. Time management skills – excellent ability to plan, prioritise and manage own workload and time in a fast paced, demanding role Ability to problem solve, work under own initiative and able to work under pressure. Possesses a ‘can do’, positive attitude. ​ Other Full, current driving licence and use of a car which is insured for business use is an essential requirement of this role. Ability to communicate through the medium of Welsh. ​
