The role of a Social Prescriber Link Worker is to provide a diverse range of support services to people who are referred to the service through GP Practices. The post holder will empower individuals to take control of their health and wellbeing through referral to non‑medical ‘link workers’ who give time, focus on ‘what matters to me’ and take a holistic approach, connecting people to community groups and statutory services for practical and emotional support.
The role will work across the TWNS (Tewkesbury, West Cheltenham, Newent and Staunton) and Cheltenham Central Primary Care Network. This is a bank role, to help cover staff absences.
Duties
* Taking referrals from health care professionals and referrals from a wide range of agencies, working with GP practices, pharmacies, multi‑disciplinary teams, hospital discharge teams, allied health professionals, police, job centres, social care services, housing associations and voluntary, community and social enterprise (VCSE) organisations.
* Working within communities, to establish needs based on a holistic assessment and contribute to forming a robust action plan agreed with the individual and other key agencies.
* Assisting individuals to take decisions on what type of support they require that best meets their health and wellbeing needs. This will involve using a range of behavioural change techniques such as motivational interviewing, health coaching and strength‑based approaches to determine their skills, interests and motivation.
* Co‑producing and implementing strength‑based Personal Development Plans based on their priorities, interests, values and motivations – including what they can expect from the groups, activities and services they are being connected to and what they can do for themselves to improve their health and wellbeing.

Position Details

Hours: Bank hours to help cover staff absences

Pay: £12.60 per hour
