Detailed job description and main responsibilities · The post holder will assist patients with their personal, physical and emotional needs, within the overall programme of care as directed by the registered professional. · To undertake nursing, therapy and social support activities following training and demonstration of competency, delegated by the relevant profession. · To work without direct supervision undertaking tasks delegated by the relevant profession in order to support the team. · To deliver high quality care to patients within the home and clinic setting, respecting patients. · To ensure that consent is obtained before any intervention is implemented. · Respect the patient and carers as individuals always respecting privacy and dignity. · Participate in administering medication, hard collar care, stoma care and eye drops. This list is not exhaustive, but all training and competencies will be offered. · Provides advice which contributes to the care, wellbeing or education of patients, including Health Promotion. · Maintain confidentiality of all information gained during course of duty. · Work flexibly to meet individual health care needs of the patient and carer.
