Assistant Practitioner - Community Falls Team
An exciting opportunity has arisen for a motivated individual to join the Community Falls Team working 12 hours per week.
The specialty of the falls service is to offer high-quality, evidence-based practice to identify those at risk of falls and opportunities to employ early interventions. Knowledge and understanding of falls prevention are desirable.
Main duties of the job
Applicants need to be flexible in work bases, including Wrexham and Flintshire, and should be able to work well both in a team and independently.
The role includes offering guidance and advice to Care Homes in Wrexham and Flintshire, following pre-determined training programmes as identified by the Community Falls Lead.
Effective communication with MDT partners is essential, including delivering pre-prepared training sessions to small groups of Care Home staff.
Speaking Welsh is desirable; applications from English and Welsh speakers are equally welcome.

Person Specification
Qualifications and / or Knowledge
* Level 4 vocational qualification in a relevant health care-related subject or Certificate in Health Care Support Worker Education
* GCSE Maths and English Grade C or above, or equivalent Level 2 or above qualifications
* Evidence of ongoing personal development relevant to healthcare
* Understanding of the HCSW Code of Conduct, All Wales Delegation guidelines, and the Band 4 Assistant Practitioner Governance Framework
* Relevant clinical knowledge
* Understanding of procedures and treatment plans
* Knowledge of legislation related to Safeguarding, the Mental Capacity Act, and Deprivation of Liberties Safeguards
* Knowledge of local statutory and voluntary services
* Teaching or assessment qualification
Other
* Flexibility to work hours including weekends, bank holidays, early, late, and night shifts
* Self-motivated, proactive, and resourceful; capable of motivating others
* Ability to travel as needed
* Willingness or ability to learn Welsh
Experience
* Recent relevant clinical care experience
* Strong interpersonal and communication skills, including managing challenging situations
* Experience with clinical skills such as catheter care, vital sign monitoring, blood glucose interpretation, wound care, and supporting RNs
* Experience in community or primary care settings
* Administration of medicines
* Basic IT skills
* Compassionate attitude and effective communication
* Ability to work within multi-agency teams and independently as an Assistant Practitioner
* Strong organizational and prioritization skills
* Ability to undertake patient assessments within competency and protocols
* Reflective practice and clinical supervision engagement
* Experience in supervising and mentoring learners and colleagues
* Excellent verbal and non-verbal communication skills

This role requires a DBS check due to the Rehabilitation of Offenders Act (Exceptions Order) 1975.
Salary: £26,928 to £29,551 per annum, pro-rata.
