Salary: £25,989-£28,142 per annum | Permanent, Full Time, 37 hours per week | Care Navigation Service | Preston

An exciting opportunity has become available for an enthusiastic and motivated person to join our growing Care Navigation Service.
In this rewarding role, you’ll be part of a supportive team of caring and compassionate colleagues, working closely with internal teams and external care providers each day. Using a range of ICT systems, software, and telephone communication, you will help secure vital packages of care for residents across Lancashire.
This is a fast‑paced role, so you’ll need to be highly organized, able to balance competing priorities, and maintain accuracy and attention to detail while working to tight deadlines.
Development: No previous sourcing experience is required. Full training will be provided, along with ongoing support to help you develop the skills needed to succeed in the role.
Key Responsibilities
* You will be responsible for sourcing care packages with external care providers for Lancashire residents using relevant IT systems and software and completing all associated administration tasks.
* You will have an allocation of work to manage independently.
* The role requires excellent verbal and written communication skills, enabling you to engage effectively with internal and external customers and colleagues at all levels. You will use a range of communication methods including telephone, electronic, and other media.
* Demonstrate the ability and knowledge to follow established processes and procedures, and to operate effectively within relevant legal frameworks.
* Adhere to strict timescales and elevate cases where necessary.
* Maintain timely, accurate, and up‑to‑date information and electronic records for management and other professionals, ensuring all data is handled securely.
* Work as part of a busy team and in collaboration with colleagues to ensure service targets are met.
* Work with limited supervision using creative thinking to solve problems.
* Develop excellent working relationships with both internal and external colleagues.
* On a rota basis – cover miscellaneous tasks such as allocating, dealing with team mailbox queries, and updating and distributing regular service information.
* Work closely with colleagues, managers, and other professionals to deliver an effective and high‑quality service.
Further Information
* Full‑time permanent officers are sought.
* Successful applicants will be based in the County Hall Complex in Preston.
* The service operates 364 days a year (closed on Christmas Day). Candidates will participate in a rota to cover working hours between 8 am – 6 pm Monday to Friday, as well as weekend and bank holiday work between 9 am – 5 pm.
