Overview

Don’t miss out on this fantastic opportunity to join Rushcliffe Community Therapy Service as an Occupational Therapist working in the community. If you want to be part of a forward thinking and innovative service, then this is the opportunity you may have been looking for.

You will work with Physiotherapists, Assistant Practitioners, support staff, district nurses, specialist nurses and social workers to provide the best possible care to service users in their own homes.

We are working towards a 7 day service – 8am to 8pm on rota basis. Self rostering is encouraged.

An exciting opportunity has arisen to join our developing integrated community services providing clinical triage, rehabilitation, falls management, palliative care support and support timely discharges from our acute Trust partners.

You will work autonomously as well as part of the multi-disciplinary team, liaising closely with health and social care colleagues to coordinate services, promote recovery from illness and enable health, wellbeing and self-care. You will be visiting patients within their own homes using a goal-orientated focus, to maximise patient’s independence, safety and quality of life. The post will involve cross site working and community visits.

A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Responsibilities

* Provide Occupational Therapy within the community, visiting patients in their own homes using a goal-oriented approach to maximise independence, safety and quality of life.
* Work closely with multi-disciplinary colleagues (Physiotherapists, Assistant Practitioners, nursing staff and social workers) to coordinate services and promote recovery from illness.
* Contribute to integrated community services including clinical triage, rehabilitation, falls management, palliative care support and timely discharges from acute Trust partners.
* Operate autonomously as well as part of the MDT, with cross-site working across community settings.
* Adhere to rota for extended hours (7 days: 8am–8pm) with self-rostering encouraged.
* Meet the driving requirement for the role and comply with Equality Act 2010 adjustments where applicable.
