Overview

The post holder will undertake assessments of service users’ needs, working in partnership with service users and their families, to formulate personalised care and support plans and risk assessments. The post holder will work with colleagues and allied resources to review the effectiveness of interventions and re-plan using evidence-based interventions in partnership with the service user. The post holder will be involved in physical procedures, including administration of oral and injectable medications, storage and transportation of medications, and monitoring of physical responses (e.g., blood pressure, glucose, weight). The post holder will carry a clinical caseload and act as the lead practitioner fulfilling the named nurse role for a group of service users. The post provides opportunities to specialise in different aspects of specialist mental health care. The post holder will undertake comprehensive personalised care and support needs and risk assessments (including capacity assessments) and, where necessary, administer prescribed medication or supervise its administration and assess the service user for desired and non-desired effects. The post will require maintaining robust clinical records and preparing reports as required, and being aware of and compliant with relevant mental health and work-related legislation and policy at all times.

Responsibilities

* Undertake assessments of needs and develop personalised care and support plans and risk assessments in partnership with service users and families.
* Review effectiveness of interventions and re-plan using evidence-based approaches in collaboration with service users.
* Carry out physical procedures including administration of prescribed medications (oral and injectable), storage/transportation, and monitoring of physical health indicators.
* Maintain a clinical caseload and act as lead practitioner (named nurse) for a group of service users.
* Specialise in different aspects of specialist mental health care as appropriate.
* Maintain robust clinical records and prepare required reports; comply with relevant mental health and work-related legislation and policy.
* Provide care in community settings and work with a multidisciplinary team and wider local mental health system.


Qualifications

* RMN or Dip / BSc (Hons) in Mental Health Nursing
* ENB 998 / Preceptorship and Mentorship
* Professional registration with UK body
* Up-to-date knowledge of relevant legislation, ethical issues, strategic frameworks and guidelines for adult mental health
* Evidence of continuing professional development
* Clinical risk assessment and management in a community setting
* Ability to communicate effectively within a multi-professional team and with diverse communities


Desirable criteria

* ENB 812 / BSc in Specialist Community Nursing Practice (Mental Health)
* Psychosocial interventions; experience across relevant settings
* At least 2 years post-registration experience with at least 1 year at Band 5; experience in acute mental health inpatient/day care; crisis and emotionally demanding environments
* Experience and knowledge of safeguarding, teaching/training, and working in a multi-ethnic community
* Experience delivering evidence-based care; knowledge of parenting literature and its application
* Ability to teach using multimedia materials; knowledge of current research developments involving service users; supervision skills
* Skills in complex assessment and treatment of individuals, couples and families; ability to apply theory to practice and formulate community care plans
* Ability to work across service boundaries and manage organisational stress
* Excellent IT skills; ability to communicate clearly in writing and verbally with various stakeholders

* Organisational skills: ability to maintain high standards of record-keeping and data entry; work autonomously in line with professional, ethical guidelines and Trust policies
