An exciting opportunity has arisen within the Nairn Integrated Health and Social Care Team. We are looking for a Band 4 Health and Social Care Coordinator (HSCC) to join our team on a fixed term basis for 32.10 hours per week, 5 days a week (Mon-Fri).
Responsibilities
* The HSCC acts as the first point of access to Health and Social Care services within the district, including Social Work, Care at Home, Allied Health Professionals and Community Nursing.
* Work daily with the Integrated Team and provide a service to the Nairn population and other Health and Social Care Professionals.
* Gather referral details from clients, family members and other Health and Social Care Professionals; share information with the multi-disciplinary team to prioritise and allocate resources.
* Provide support to professional staff when dealing with urgent and critical cases in the community.
Qualifications and Skills
* Strong communication and organisational skills.
* Qualifications and/or experience in Health and Social Care.
* Ability to work in a collaborative, team-based environment.
Requirements
* Full driving licence and use of car for work purposes is essential.
