STEPs Community Reablement Officer (CDC)
Based at: Holmescarr Centre, Rossington, Doncaster
Salary details: Grade 6, £26,403 - £27,255 pro rata, for 25 hours + All Inclusive Allowance (pay award pending)
Contract Type: Permanent
Hours: Part-time, 25 hours per week, working a 14 day rota pattern on evenings.
Enhanced Level DBS Disclosure is required for appointment to this post.

Job description
The City of Doncaster Council is a confident, ambitious organisation which puts improving the life of its residents at the centre of everything we do. You have an opportunity to be part of this in the role of STEPS Community Reablement Officer.
STEPs team (short term enablement programme) need Community Reablement Officers that can demonstrate the 6 C’s: Care, Compassion, Competence, Communication, Courage, Commitment.
You will support people in their own home for up to 6 weeks following a hospital stay or to prevent a hospital admission. You will assist and support people to live how they chose in a safe environment while promoting independence with practical support as agreed in their support plan, this is called reablement.
Responsibilities
* Providing personal care.
* Assisting people with mobility transfers following a movement and handling plan.
* Providing support with medication and meals.
* Understanding safeguarding and risk assessments.
* Working within a 14‑day rota pattern on evenings.
You will need a full driving licence and own transport to travel throughout the borough.
