About the Trust Locations

The Countess of Chester Hospital NHS Foundation Trust provides services to West Cheshire and Welsh patients covered by Betsi Cadwaladr University Health Board. The Trust works collaboratively within the wider Cheshire and Merseyside Health and Care Partnership. Its services are provided from three locations: The Countess of Chester Hospital: providing 438 general and acute beds; Ellesmere Port Hospital: providing 56 beds as a rehabilitation, intermediate and outpatient facility; Tarporley War Memorial Hospital: a base for community services which serve the local rural population.


About the Role

We are looking for a Rehab Assistant to work within the Rapid Response Team, providing an early supported discharge service to patients from The Countess of Chester Hospital & Ellesmere Port Hospital. You will be required to work shifts over 7 days between the hours of 8am and 9pm. You will be lone working across the West Cheshire Community. A full driving licence and car are required for this role. Recruitment selection processes are based on competence (see Person specification) and values.


Responsibilities

* Assist patients with their activities of daily living, providing resettlement, rehabilitation & care to patients in their own homes, delivered in accordance to the patient's Care Plan.
* Support therapists and nurses with their patient caseload, manage a caseload of your own under the supervision of qualified staff & Senior Community Support Workers, and perform general administrative and housekeeping tasks as required to facilitate the smooth running of the service.
* Exhibit excellent interpersonal skills, be self‑motivated, empathetic and able to develop practical skills. It is essential to have a holistic view of patient care, with a focus on facilitating patient safety & independence in their own home.
* Provide a service 7 days per week including bank holidays and evening shifts.


Requirements

* Full driving licence and own car.
* Ability to work independently and manage a caseload.
* Strong interpersonal and communication skills.
* Empathy and self‑motivation.
* Holistic view of patient care with focus on safety and independence at home.
* Valid DBS check (Applicants will be required to pay for their DBS check; costs will be deducted from salary over the first three months of employment).


Additional Information

* Applicants may be required to enrol for the DBS Update Service; a small annual fee of £16 per year applies.
* New entrants to the NHS will commence on the minimum of the scale stated above.
* Applicants are advised to apply early; if a large number of applications are received we reserve the right to close the vacancy prior to the advertised date.
* Applicants must meet the Disclosure and Barring Service (DBS) requirements; a submission will be made if the role falls under the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020.
* Enhanced DBS applications with omitted or incorrect information will be withdrawn; any errors will require a new application and a payment of the DBS application fee again.
* The cost of the DBS application will be deducted from the salary over the first three months of employment.
* Overseas candidates: The Department of Health and Social Care has released a new publication to support overseas candidates looking to work in health and care in the UK.
