Advanced clinical practitoner

The post holder will be an Advanced Clinical Practitioner who can demonstrate the following:-

* Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with undifferentiated and undiagnosed conditions and complex needs.
* Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
* Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).
* Post registration experience in at least one of the following areas, relating to the service applying for: Out of Hours Urgent Primary care, General practice, walk in centre, A&E.
* Ability to see patients of all ages
* Willing to undertake predominantly face to face with telephone consultations when required.
* Ability to conduct remote consultations/assessments
* Preferred skill experience in Emis.


Main duties of the job

* Patient Assessment:Conducting thorough physical exams, taking medical histories, and ordering diagnostic tests.
* Diagnosis and Treatment: Making independent diagnoses, developing treatment plans, and prescribing medications within their scope.
* Chronic Disease Management: Managing chronic conditions like diabetes and COPD, working with patients to achieve optimal health.
* Acute Care: Providing care for acute presentations and minor health problems.
* Referrals: Making appropriate referrals to specialists or organizing emergency admissions when needed.
* Investigations: Ordering necessary blood tests, radiology, and other investigations.

Job responsibilities

The Advanced Clinical Practitioner(ACP) has beenassessed as competent in practice using their expert clinical knowledge andskills. They have the freedom and authority to act and make autonomousdecisions in the assessment, diagnosis and treatment of patients.

The post holder works at the level of advanced clinicalpractice and has developed and can evidence the underpinning competenciesapplicable to the specialty or subject area, i.e., the knowledge, skills andbehaviours appropriate to the healthcare professionals (HCP) setting and jobrole.

The role provides an opportunity for patients to avoid theneed to be referred to a GP, thereby enabling them to receive timely care andnegate unnecessary delay in receiving treatment.

An ACP is autonomous in making decisions based onassessment, diagnosis and interpretation of test results and will initiate,evaluate and modify a range of interventions which may include prescribingmedicines, therapies, lifestyle advice and care.

The following are the core responsibilities of the AdvancedClinical Practitioner.

There may be, on occasion, a requirement to carry out othertasks; this will be dependent upon factors such as workload and staffinglevels:

* Practise in compliancewith their respective code of professional conduct and within their scopeof practice, being responsible and accountable for their decisions,actions and omissions at this level of practice
* Demonstrate a criticalunderstanding of their broadened level of responsibility and autonomy andthe limits of own competence and professional scope of practice, includingwhen working with complexity, risk, uncertainty and incomplete information
* Act on professionaljudgement about when to seek help, demonstrating critical reflection onown practice, self-awareness, emotional intelligence, and openness tochange
* Work in partnership withindividuals, families and carers, using a range of assessment methods asappropriate
* Demonstrate effectivecommunication skills, supporting people in making decisions, planning careor seeking to make a positive change
* Use expertise anddecision-making skills to inform clinical reasoning
* Initiate, evaluate andmodify a range of interventions which may include prescribing medicines,therapies, lifestyle advice and care
* Exercise professionaljudgement to manage risk
* Manage patients presentingwith a range of acute and chronic medical conditions, providing subjectmatter expert advice
* Implement and evaluateindividual specialised treatment plans for chronic disease patients
* Identify, manage andsupport patients at risk of developing long-term conditions, preventingadverse effects to the patients health
* Be a professional rolemodel for the organisation
* Actively engage in peerreview to inform own and others practice, formulating and implementingstrategies to act on learning and make improvements
* Actively seek feedback andinvolvement from individuals, families, carers, communities and colleaguesin the co-production of service improvements
* Critically assess andaddress own learning needs, negotiating a personal development plan thatreflects the breadth of ongoing professional development across the fourpillars of advanced clinical practice (clinical practice, leadership andmanagement, education and research).
* Provide chronic diseaseclinics, delivering patient care as necessary, referring patients tosecondary/specialist care as required
* Maintain accurate clinicalrecords in conjunction with extant legislation and to ensure SNOMED CT codes are used effectively
* Chaperone patients wherenecessary
* Support the team indealing with clinical emergencies
* Support the team with allsafeguarding matters in accordance with local and national policies
* Understand practice andlocal policies for substance abuse and addictive behaviour, referringpatients appropriately
* Deliver opportunistichealth promotion where appropriate


Person Specification


Knowledge and Skills

* Understand the importance of evidence based practice
* Broad knowledge of clinical governance
* Ability to record accurate clinical notes
* Ability to work within own scope of practice and understand when to refer to GPs
* Knowledge of health promotion strategies
* Understand the requirement for PGDs and associated policy
* Polite and confident, flexible and cooperative
* Motivated, forward thinker
* Problem solver with the ability to process information accurately and effectively, interpreting data as required
* High levels of integrity and loyalty
* Sensitive and empathetic in distressing situations as well as ability to work under pressure/in stressful situations


Qualifications

* Educated to Masters degree as detailed within the HEE document tilted Multi-professional framework for advanced clinical practice in England
* Qualified independent prescriber - V300
* Registered on their appropriate professional register


Experience

* Experience of practice within the four pillars.
* Job plan that demonstrates advanced clinical practice and has equity with peers working at this level
* Experience of prescribing and undertaking medication reviews
* Experience of working in a primary care environment
