This patient pathway Co - Ordinator for hepatology will be responsible for supporting the care of patients living with liver cirrhosis through the implementation of a nationally recognised database to identify and track individuals requiring treatments to detect Hepatocellular carcinoma.
This role will enable the hepatology service to continue to deliver exceptional patient care and meet minimal national standards, as outlined in the "NHS England Hepatocellular carcinoma (HCC) surveillance: minimum standards" guidelines.
The pathway coordinator will support the patient through the journey from identification of cirrhosis and referral to conclusion of their intervention, thus eliminating barriers to care.
The pathway coordinator will work closely the wider gastroenterology MDT to ensure a patient focussed service and will keep accurate patient and activity records to facilitate reporting back to commissioners regarding performance and service success.
Main duties of the job
To support the trust to achieve the national minimum standards of Hepatocellular Carcinoma (HCC) Surveillance.
Supporting the implementation of a nationally recognised HCC Surveillance database with the trust.
Monitoring and managing the HCC surveillance data base ensuring it is up to date.
Act as centralised point of contact for patients. Proactivity contacting patients to provide support.
Working with the hepatology team to support workstreams that optimise the care and health of liver patients who access the hepatology services.
Support the reporting of data to local and national commissions and the cancer alliances in a robust and timely way.
Coordinate patent's abdominal ultrasound scans, blood tests and clinical reviews in a timely manner and ensuring results are evaluated by the hepatology clinical team.
Liaise with different departments within the trust and externally to the trust to ensure patient care is met.
To help reduce patient cancellation and non-attendances for tests and clinic appointments influencing rapid surveillance pathway.
To undertake a range of administrative duties which facilitate and support coordination of the surveillance programme including assisting in arrangement of appointments, tests and interventions.
To escalate issues with the surveillance programme in a timely fashion.
To actively engage in service improvement to maximise patient experience and outcomes.
To participate in local and national audits as required.

EDUCATION, QUALIFICATIONS & TRAINING
Essential criteria
1. Good general education with English and Maths to GCSE standard or equivalent.
2. Educated to diploma level or NVQ 3, City and Guilds certificate level in business administration or equivalent
Desirable criteria
3. Customer services qualification/training.
4. IT package qualification/training, e.g. European Computer Driving Licence (ECDL).
EXPERIENCE
Essential criteria
5. Significant proven experience of delivering to high standards in a fast-paced high-volume customer care environment.
6. Ability to deal with a range of issues independently and efficiently.
Desirable criteria
7. Experience of an administrative role within the NHS
SKILLS, ABILITIES & KNOWLEDGE

8. Excellent interpersonal skills with the ability to liaise effectively with a wide range of multi-disciplinary staff.
9. Knowledge of medical terminology.

12. Ability to work with confidential data and information and deal with sensitive issues discreetly.

SPECIAL CIRCUMSTANCES
Essential criteria
14. Able to work across sites at both Stoke Mandeville hospital and Wycombe hospital.
