What you will do: Conduct comprehensive assessments within the community for individuals with permanent or complex disabilities, ensuring the provision of services that prevent ongoing dependency. Perform assessments in accordance with relevant legislation including the Care Act and Housing Grants, Construction and Regeneration Act, to reduce handoffs while ensuring environmental changes bolster independence. Collaborate within a multidisciplinary team to appraise the effectiveness of current care provisions, finding innovative ways to promote self-reliance among service users. Oversee and guide the work of Occupational Therapy Assistants and students, fostering a culture of learning and improvement. Participate in initiatives within the OT and Sensory service that advance the aspirations and vision of Adult Social Care. Engage constructively with service users, carers, and other stakeholders to empower individuals, maximising their independence.
